We learn about your service, your people, your strengths and what truly matters to you.
Independent assurance, evidence review and practical improvement. We look beyond policies and forms to see what's actually happening — and help you fix what can be fixed.
Same reliable process every time. No hidden agenda.
We learn about your service, your people, your strengths and what truly matters to you.
We look at records, observe practice and speak with people respectfully — staff, families and those using services.
We show what the evidence supports: what is working, what needs attention and what remains missing or unclear.
Together we turn findings into practical priorities, clear ownership and realistic next steps.
Where agreed, we stay involved through check-ins, learning and follow-up review so progress has a chance to stick.
Timescale fits the work. Not a one-size deadline. Scope confirmed before we start. No surprises.
A family, frontline worker, manager, social worker, commissioner or regulator may each legitimately need different information about the same situation. ORVIA helps bring the underlying material into a traceable picture and prepare the relevant information for the people who need to act.
Not everybody sees everything. The right people should have the information they legitimately need, with the source and context still traceable.
We do not start by deciding who is at fault. If an incident involves individual error, the evidence should show it. If handover, training, staffing, conflicting information or system design contributed, that should be visible too. If both contributed, both should be addressed.
ORVIA reviews the evidence and the operating picture. Employers, statutory bodies and other lawful decision-makers retain their own authority and responsibilities.
Every route expresses the same ORVIA foundation: Observation · Reflection · Visibility · Insight · Accountability.
Scattered notes, conflicting accounts or material that needs to stand up to scrutiny.

A verbal feedback session to ask questions, challenge findings and agree what happens next.
Traceable, honest and evidence-based — something you can stand behind and explain to others.
Check-ins, practice learning and follow-up review. We do not hand it over and disappear.
Four Realities. One clearer answer.
VITA does not just check whether a requirement appears to be met. It compares what is said, what is written, what is seen and what people actually experience — then makes the gaps visible.
Scope a VITA reviewAn honest independent check: are things as they should be?
Not simply “is it written down?” but “does the available proof support it?”
What happens day-to-day — not just what the policy says should happen.
Who knew what, when, and what did they do about it?
Independent enough to challenge. Supportive enough to help improve.
A clear record of what we reviewed, what we found and what the finding is based on.
Just tell us what's happening — we'll guide you.
